With the end of the school year fast approaching, please check your child's shot records to be sure that they are current on all their shots. If you do this before school starts next year you could get any needed shots during the summer when they are out of school and their records would be complete when starting in August.
I have listed some information that you may find helpful in the upcoming school year.
You will find the shot requirements listed here for your information and to help you know the shots your child will need to have.
You will also find a medication authorization form that you can print and take to the doctor for them to fill out for your child's medication that needs to be given during the school day. Such as if a medication says every 8 hours that is one that can be given at home. If it says 2 times a day you can give that one at home also.
